Adult Partial Care Referral Portal

The Adult Counseling and Therapy (ACT) Program is an out-patient partial care day program for adults living with a mental illness. Adults eligible for this program must have an Axis I diagnosis, be at least 18 years of age, demonstrate the need to qualify for partial care services, and be in our serving counties (Camden/Gloucester). Referrals are accepted from any source, and Medicaid is the primary source for payment. To see if the ACT Program is the right fit for you or your consumer, please call us at 856.582.3900. You can also make an online referral by clicking on the ‘Adult PC Referral Form’ below.
