Support Coordinators Referral Portal

As parents and caregivers of a loved one with an intellectual/developmental (I/DD) disability, we understand you are required to provide care 24 hours a day, 7 days a week, 365 days a year. Archway has over thirty years of expertise serving individuals with I/DD. Our staff is committed to providing individualized care and will advocate for your loved one, ensuring they receive the highest quality of care. Families now have the ability to choose services for their loved ones. Support coordinators receive referrals from Division of Developmental Disabilities (DDD). Archway Programs is an approved fee-for-service (FFS) Medicaid Provider and is accepting placement/service for referrals. Adult Partial Care Referral Portal

The Adult Counseling and Therapy (ACT) Program is an out-patient partial care day program for adults living with a mental illness. Adults eligible for this program must have an Axis I diagnosis, be at least 18 years of age, demonstrate the need to qualify for partial care services, and be in our serving counties (Camden/Gloucester). Referrals are accepted from any source, and Medicaid is the primary source for payment. To see if the ACT Program is the right fit for you or your consumer, please call us at 856.582.3900.
